Eat Well offers healthy meals to enrollees. Your care manager will specify how many healthy meals you are eligible to receive.

Meals are delivered directly to a patient’s home via FedEx or another delivery service.

  • Meal service allows patients to choose what they want to eat.

  • Meals are designed by dietitians to meet nutritional requirements and are prepared with high-quality ingredients in USDA-approved kitchens.

  • Patients receive 28 refrigerated meals every two weeks for as long as they are eligible.

  • Meals last 14 days and can be heated in 2 minutes or less.

Who is eligible to receive healthy meals from Eat Well?

Must be a Medicaid managed care member enrolled in Standard Plan or enrolled in I/DD Tailored Plan.

  • Must live in pilot region.

  • Must have at least one physical or behavioral health criteria:

    • Adults with two or more qualifying chronic conditions.

    • Pregnant women with high risk of complications.

    • Child under 3 years old who graduated from neonatal intensive care unit.

    • Child under 20 years old experiencing three or more categories of adverse childhood experiences.

  • In addition to one physical or behavioral health criteria, must have one social risk factor:

    • Unhoused and/or housing insecure

    • Food insecure

    • Transportation insecure

    • At risk of, witnessing, or experiencing interpersonal violence

How can I receive healthy meals from Eat Well?

Your care manager will determine if you meet eligibility criteria and will assist you with enrollment. If you think you may be eligible, call the phone number on the back of your insurance card and let them know you’d like to join the Healthy Opportunities Pilot. Once enrolled, you can request healthy meals from Eat Well.

Meals are delivered straight to your door, using FedEx or another national delivery service.

How often do I get healthy meals from Eat Well?

Fourteen refrigerated meals are delivered each week.

How long does the program last?

You will receive healthy meals for as long as your prescription indicates.
