Eat Well

Eat Well offers healthy food boxes to CCAH Medi-Cal members with chronic health conditions.

This service is available to eligible CCAH members with chronic conditions, such as but not limited to:

1. Diabetes, cardiovascular disorders, congestive heart failure, stroke, chronic lung disorders, human immunodeficiency virus (HIV),cancer, gestational diabetes, or other high 55 risk perinatal conditions, and chronic or disabling mental/behavioral health disorders.

2. Individuals being discharged from the hospital or a skilled nursing facility or at high risk of hospitalization or nursing facility placement; or

3. Individuals with extensive care coordination needs.

Healthy food boxes are delivered via mail directly to a patient’s home.

  • Boxes can reflect patient dietary needs as well as preferences.

  • Patients have access to healthy & primarily organic foods.

  • Patients receive one box per week for up to 12 weeks with option to renew.

Who is eligible to receive healthy food boxes from Eat Well?

  • Must be a Medi-Cal CCAH member with a one or more of the chronic health care conditions listed above.

  • Must live within Santa Cruz, San Benito, Monterey, Mariposa or Merced counties.

  • Must have at least one physical or behavioral health criteria:

    • Patients with two or more qualifying chronic health care conditions.

    • Unhoused and/or housing insecure

    • ECM Member

How can I receive healthy food boxes from Eat Well?

Yourself or care manager/provider can refer using the CCAH website under medically tailored meals. CCAH will determine if you meet eligibility criteria and approve or decline referral. If approved, a member of our team will reach out to conduct initial assessment. Once enrolled, you will receive healthy food boxes from Eat Well tailored to your nutritional needs.

How do I get my healthy food box from Eat Well?

Boxes are delivered straight to your door, using FedEx or another national delivery service.

How often do I get a healthy food box from Eat Well?

Healthy food boxes are delivered weekly.

How long does the program last?

You will receive food boxes for up to 12 weeks with option to renew upon CCAH discretion.
